Note Regarding Form 62-555.900(2)

Form 62-555.900(2) is the monthly operation reports for subpart H systems.  This form is accessible at http://www.dep.state.fl.us/water/drinkingwater/forms.htm.

Certain requirements of the federal Long Term 1 Enhanced Surface Water Treatment Rule (LT2ESWTR) and associated state rules changed on January 1, 2005.  The requirements pertain to subpart H systems serving less than 10,000.  The current form has an effective date of October 14, 2004 and is still usable by subpart H systems serving 10,000 or more.  Systems serving less than 10,000 are requested to be aware that the Department plans to conduct rulemaking in early 2005 to update this form.  In the interim the Drinking Water Section will send all systems serving less than 10,000 an interim copy of Form 62-555.900(2) to be used until the updates are adopted into the Florida Administrative Code.
